Entry Fee & Waiver Forms

 

The following Entry Fee and Waiver Forms must accompany your payment. All entries, except Masters, must be done online no later than December 21, 2007. Masters pre-meet entries must be done online or postmarked no later than December 21, 2007.

 

ALL entry fees must be accompanied by a signed Entry Fee and Waiver form and must be postmarked no later than December 21, 2007.
